Licensed Clinical Professionals volunteer to visit dialysis clinics throughout Santa Clara County to:

• Notify Dialysis Social Workers about current community interventions to save as many lives as possible from the impact of H.R. 1. 

• Encourage staff to submit questions and concerns about how the severe cuts to Medicaid, Medicare, and safety-net services will impact them.

• Encourage staff to sign up for our email notifications.

• Encourage staff to sign up as Outreach Volunteers.

• Ask staff to encourage patients and family caregivers to submit questions and concerns and sign up for our email notifications.

Licensed Clinical Professionals volunteer to visit County Hospitals throughout Santa Clara County to:

• Notify Social Workers, Case Managers, and other clinical staff about current community interventions to save as many lives as possible from the impact of H.R. 1.

• Encourage staff to submit questions and concerns about how the severe cuts to Medicaid, Medicare, and safety-net services will impact them.

• Encourage staff to sign up for our email notifications.

• Encourage staff to sign up as Outreach Volunteers. 

• Ask staff to encourage patients to submit questions and concerns and sign up for our email notifications.

• Speak directly with patients and their family caregivers to submit questions and concerns and to sign up for our email notifications.

Clinical and Non-Clinical Outreach Volunteers attend rallies, townhall meetings, and organizing meetings throughout Santa Clara County to:

• Notify Santa Clara County residents about current community interventions to save as many lives as possible from the impact of H.R. 1.

• Encourage residents to submit questions and concerns about how the severe cuts to Medicaid, Medicare, and safety-net services will impact them and their families.

• Encourage residents to sign up for our email notifications.

• Encourage residents to sign up as Outreach Volunteers. 

New and existing free services based in Santa Clara County will be published on this website and on printed educational materials.

Licensed Clinical Professionals volunteer to visit postacute care facilities throughout Santa Clara County to:

• Notify Social Workers and Case Managers aabout current community interventions to save as many lives as possible from the impact of H.R. 1.

• Encourage staff to submit questions and concerns about how the severe cuts to Medicaid, Medicare, and safety-net services will impact them.

• Encourage staff to sign up for our email notifications.
